The Health Insurance
Portability and Accountability Act (HIPAA) Privacy Rule describes
privacy standards that protect your medical and health information.
It identifies your rights to access and control your health information.
It outlines how various health providers may use and disclose your
protected health information to carry out treatment, payment or
health care operations and for other purposes that are permitted
or required by law. Effective April 14, 2003, all health care facilities
and physicians will be required to comply with the new federal privacy
rules concerning how your health information is shared, stored and
utilized. Our Surgical Center considers protecting your privacy
an important part of caring for you. If you have any questions regarding

for download and printing if you wish. They are in Adobe Acrobat
format. If you do not have Adobe Acrobat Reader 5 or higher, you

is the actual HIPAA Privacy Practices. The RECEIPT is an acknowledgement
that you have received the Notice. The CONSENT is needed for use
and disclosure of health information. Feel free to review these
documents, print them, sign them and hand them in at the time of
your procedure.
